Each year on April 16, National Healthcare Decisions Day serves as an important reminder for individuals and families to have meaningful conversations about future healthcare preferences. Advance care planning is the process of thinking through, discussing, and documenting the care you would want if you were unable to speak for yourself—and it is one of the most empowering steps a person can take in managing their health.

At its core, advance care planning ensures that your values, goals, and wishes guide medical decisions. By completing documents such as a living will or designating a healthcare surrogate, individuals provide clarity to loved ones and healthcare providers during what can be emotionally challenging situations. This preparation reduces uncertainty and stress for families, who might otherwise be left guessing about critical decisions.

Beyond easing the burden on others, advance care planning improves the quality of care you receive. When providers understand your preferences, whether related to life-sustaining treatments, comfort-focused care, or specific cultural or spiritual considerations, they can align treatment plans accordingly. This leads to more personalized, respectful care that honors your priorities.

Important to note is advance care planning is not just for older adults or those with serious illness. Even if you are young and healthy, unexpected health events can happen, making it essential for all adults to have a plan in place. These conversations can evolve over time as circumstances and perspectives change, making it a dynamic and ongoing process rather than a one-time task. National Healthcare Decisions Day encourages everyone to start the conversation, document their wishes, and share them with those who matter most.

Utilizing a tool such as Five Wishes® makes this an easier process, too. Five Wishes is a widely known document that is easy to follow, understand, and complete. It provides the framework that covers medical, emotional, spiritual and communication wishes to make decision-making easier for family and loved ones in the event you are unable to speak for yourself. It is available in 30 languages and is a legally valid document in nearly every state. Taking these steps today can bring peace of mind tomorrow ensuring that your voice is heard.
