To help individuals experiencing dementia or cognitive impairment and the caregivers who support them, VNA of the Treasure Coast, in collaboration with Harmonic Health, is now an approved provider of the GUIDE (Guiding an Improved Dementia Experience) Model—a Medicare program designed to provide personalized dementia care and support.
Created by the Centers for Medicare & Medicaid Services (CMS), the GUIDE Model helps people experiencing dementia or cognitive impairment remain as safe, independent, and supported as possible at home.
You do not necessarily need a formal dementia diagnosis to be considered for the program. Each individual is evaluated to determine whether they meet the program’s eligibility requirements and can benefit from GUIDE services.
Through the program, eligible participants have access to a dedicated Dementia Care Navigator who can:
- Coordinate care and develop a personalized care plan
- Answer questions and provide ongoing guidance
- Connect families with local resources, including transportation, meal programs, and home safety services
- Provide education and support to caregivers
- Help families understand dementia and cognitive changes, communication strategies, behavior changes, home safety, and planning for future care
Caregivers also receive meaningful support. Eligible participants may receive $2,500 per year in Medicare-covered in-home respite care, giving caregivers time to attend appointments, run errands, spend time with family and friends, or simply rest and recharge.
