You’ll receive a copy of Five Wishes, an easy-to-use legal medical advance directive that addresses your needs and desires should you become ill and are no longer able to make decisions for yourself.
This FREE workshop will walk you through the Five Wishes booklet and help you understand the importance of advance care planning. Refreshments will be provided.
